New Client Forms
If you are a new client, please fill out these three forms and bring them with you to the first appointment. If you need me to coordinate care with any other health care professionals, there is a release of information that you can fill out in person during session.
If you are a parent bringing your child for treament, please also fill out the Children: Developmental History form.
